Text Message Notification OPT-IN Form
To better serve you, the pharmacy offers an automatic text notification service to notify you
when your medications are ready for pickup.
If you are interested in receiving text message notifications, please fill out and sign the bottom
half of this form and return to a pharmacy staff member for implementation.
Important Notes:
- You may receive one or more text messages, depending on how your medication order was received by our pharmacy and insurance approval.
- Text message(s) will be sent once your prescription order is ready for pickup. Notification for orders to be filled on future date or are on backorder will be sent when ready.
- Medications will be available for pick up for 10 business days, in accordance with regulations. We will be unable to notify when these prescriptions are returned to the shelf if you have not picked them up.
By signing this form:
- I agree to receive text notifications on my cell phone when my medications are ready.
- I understand text messaging rates may apply, as defined by my cell phone provider.
- I will notify the pharmacy staff when I no longer wish to receive these notifications.
